Flexible Payroll Deduction Authorization Information

These forms allow you to contribute additional funds on a pre-tax basis through payroll deduction. This option is not available through all employers and contracts. In addition, all forms must be submitted to the payroll department. A copy should also be forwarded to the 401(k) Plan.
